KINDERGARTEN REGISTRATION FOR THE 2013-2014 SCHOOL YEAR
On-line registration was available at this website from January 7 through February 8, 2013 and is currently closed.
For parents who completed on-line registration, your child's folder will be waiting for you on Tuesday, February 26 at the Wemrock Brook School from 10:00am to 12:00 noon. You will only need to bring with you the required documents listed below.
For those who did not complete the kindergarten on-line process, registration will be held on Wednesday, February 27 at the Wemrock Brook School at 118 Millhurst Road from 10:00am to 12 noon as indicated below. Registration is time consuming so do not bring your children .

REGISTRATION FOR FIRST GRADERS NOT CURRENTLY ENROLLED
Registration for new First Grade students, who are not already enrolled in our Kindergarten program, will be held at each elementary school on Wednesday, March 6 and Thursday, March 7 from 10:00 a.m. until 12 noon. Please call the school to set up an appointment. Telephone numbers are listed below. REQUIREMENTS
PLEASE BRING COPIES OF ALL DOCUMENTS TO REMAIN ON FILE.
(Birth Certificate, Two Proofs of Residence, Health Form, Immunization Forms)
AGE:
Children born on or before October 1, 2008 for Kindergarten
Children born on or before October 1, 2007 for First Grade
PROOF OF AGE:
Only a certified birth certificate from the Bureau of Vital Statistics with a raised seal will be accepted. A hospital notice of birth or a photostatic copy is not acceptable. (Please bring a copy for us to keep on file)
RESIDENCY:
Two proofs of residence, e.g. driver's license, utility bill. (Please bring copies to be kept on file.)
MEDICAL :
State statue 18A: 40-4 states all Kindergarten students and new students entering First Grade will be required to have physicals completed before they are admitted to school. Physical examinations performed on or after September 1, 2012 will be accepted. Forms are available in each school health office and on line.
Records of the following immunizations must be presented for registration:
Four doses of diphtheria and tetanus toxoids and pertussis
vaccine (DPT), one dose of which shall have been given on or
after the fourth birthday. Alternatively, a child with any total of
five doses of DPT type vaccines will also be in compliance with
this regulation.
Three doses of Inactivated Polio Vaccine (IPV), or Oral Polio Vaccine (OPV) or a combination of both, one dose of which shall have been given on or after the fourth birthday. Alternatively, a child with any four doses of polio vaccine, spaced by a minimum of one month (twenty-eight days) will be in compliance with this requirement.
Two doses of a live measles-containing vaccine, or any vaccine combination containing live measles vaccine, such as preferred measles, mumps, rubella (MMR) vaccine, given after age 1. The second dose must be administered no less than one month from the date of the first measles-contained vaccine.
German measles (rubella) and mumps are required after the first birthday.
Hepatitis B vaccine. A three dose regime is required. In order to avoid problems in September, please have your child's Hepatitis B status updated and bring the appropriate documentation at the time of registration.
Varicella (Chicken Pox) vaccination or written laboratory evidence of immunity. A physician's or parent's written statement verifying previous Varicella infection will also be accepted. Please include specific date.
Students transferring from another country must have a documented Mantoux skin test within the previous six months.
All students transferring from another state or country must follow Monmouth County TB requirements.
The doctor's signature must be on the immunization records. If your child was not immunized against mumps and/or regular measles because he/she had the disease, documentation signed by a physician is required. Children will not be assigned to a class in September until all records are complete. (Please bring a copy for us to keep on file.)
Students without the appropriate documentation will be allowed to enter school under provisional admission as long as a completed physical, updated vaccine documentation, and/or appointment date for medical provider is submitted by June 15, 2013. Documentation must be received by the first week of school or your child may be excluded from school.

Required forms are items 2-7 and optional forms are items 8-10. The optional forms will be important for those of you who have special issues as indicated by the titles of each of the forms. MEETING FOR ALL PARENTS
To explain our Kindergarten program and program options to parents/guardians of all potential Kindergartners, we will have a presentation by members of our administrative staff on May 13th at 7:00p.m. in the Clark Mills School on Gordons Corner Road. Your children should not attend the meeting.
